A request for ABA assistance can reveal a person’s care interests, even without a diagnosis. The form collects only the contact and preference fields shown to you. Do not enter medical records, clinical notes, a child’s name or insurance member IDs.
Purpose and permission.
Request information is used to review your request, communicate with you and, within the permission you give, share a provider copy for an appropriate response. A named-provider request remains limited to that target unless you separately select optional alternatives.
Submitting a request does not enroll you in automated marketing or unrelated text messages. Optional newsletter permission is separate. Provider receipt, response, coverage and availability are not guaranteed.
Operations and choices.
Authorized operators and service providers may process information to run the request system. Browsing is available without a care request. Contact the site through Contact & corrections about your information and available choices. Identity verification may be required before access or deletion action.
