Reported information.
Services, locations, insurance participation, ages and openings may change. A profile, filter match or result position is not an endorsement, a confirmed opening or a guarantee of benefits or outcomes. Provider websites are independent destinations.
Optional requests.
A submitted request means a request has been captured only after the form confirms acceptance. It does not mean a provider has received it, agreed to serve you or booked care. Internal review and any permitted provider sharing are separate steps. Permissions shown in the form apply to that request.
Appropriate use.
Send accurate information you are authorized to share. Do not send a child’s name, medical documents, account credentials, insurance member IDs or unsolicited marketing through the forms.
